Short answer
Cortical thinning and subcortical volume loss were already present at Parkinson's diagnosis in patients with mild cognitive impairment, and progressed faster over 18 months.
What happened
The ICICLE-PD study tracked 108 newly diagnosed Parkinson's disease patients and 38 healthy controls over 18 months with 3 T MRI, measuring cortical thickness and subcortical volumes. At baseline, patients with mild cognitive impairment already showed thinning in frontal, parietal and occipital regions and smaller hippocampal and nucleus accumbens volumes. Over 18 months those patients thinned faster in frontal and temporal regions.
Why it matters
'Newly diagnosed' marks when symptoms crossed a clinical threshold, not when the disease started. Structural differences present at that moment show the process was already well under way — which is what any trial recruiting at diagnosis is actually enrolling.

While the study shows clear structural differences over 18 months, it cannot prove that cortical thinning directly causes cognitive decline, as the relationships are correlational. Additionally, because the participants were evaluated while taking their normal dopaminergic medications, the potential masking effects of these drugs on cognitive scores and brain activity cannot be completely ruled out. The study is also limited by having only a single test to assess visuospatial function, which may have restricted its ability to fully capture this specific cognitive domain.
