What they did

The researchers investigated spontaneous thinking by following 6 male patients with selective, bilateral damage to the hippocampus and 12 closely matched healthy male controls. Over 2 days, for 8 hours each day, the experimenter unexpectedly asked participants what they were thinking about at 20 specific, restful moments. The participants described their immediate thoughts, which were then analyzed for temporal focus, memory style, and whether they were visual or verbal.

What they found

Both groups let their minds wander at similar rates, but the content differed dramatically. Healthy controls mostly visualized episodic scenes, whereas patients experienced primarily verbal, factual thoughts focused on the present moment. Patients thought significantly less about the past (with an effect size of 1.1) and imagined fewer hypothetical, timeless scenarios (effect size of 2.1), instead lingering on the present (effect size of 1.7). Furthermore, controls experienced far more episodic detail and mental scenes than patients (each showing a large effect size of 2.6).
