What they did

Researchers recruited 26 patients with generalized anxiety disorder and 26 healthy controls. The anxiety patients were randomly assigned to either an 8-week mindfulness course or an active stress-management education control. Before and after the 8-week period, all participants completed a facial affect-labeling task during fMRI scans.

What they found

At baseline, patients with anxiety showed higher amygdala activation when viewing neutral faces compared to healthy controls. Following the interventions, both groups showed decreased amygdala activity, but only the mindfulness group showed increased activation in the prefrontal cortex during the labeling task. This prefrontal activation and enhanced connectivity between the amygdala and prefrontal regions correlated with reduced anxiety symptoms.

The limits

What it doesn't show

The study had a relatively small sample size, which limits generalizability. Because there was no 'no-treatment' control group, the general decrease in amygdala activity across both treatment groups could be due to repeated exposure to the task stimuli rather than the treatments themselves. Additionally, the study did not establish if prefrontal changes cause anxiety reduction or are merely a byproduct of clinical improvement.
