Does brain region loss predict memory decline in Parkinson's?

Damage to a specific brain region called the nucleus basalis of Meynert can predict whether a person with Parkinson's disease will develop cognitive decline.
Key findings
They found that patients with Parkinson's disease who had cognitive impairment at baseline had lower grey matter volume and higher tissue diffusivity in the nucleus basalis of Meynert. In the longitudinal analysis, structural and microstructural degeneration of the nucleus basalis of Meynert predicted who would develop cognitive impairment over the 36-month period. Specifically, microstructural changes were a stronger predictor of cognitive decline than structural grey matter loss alone.
Methodology
The researchers analyzed MRI data from 304 drug-naive patients with Parkinson's disease and 167 healthy control subjects. To evaluate brain structure, they measured grey matter volume and assessed tissue microstructure with diffusion tensor imaging in a subset of 84 patients and 62 controls. They monitored patients' cognitive status every 6 months for up to 36 months to see who developed cognitive impairment.
Limitations
Because the study relies on database neuroimaging, it cannot definitively prove a causal biological mechanism without direct post-mortem tissue examination. Additionally, the study does not clarify exactly when degeneration of this brain region begins, nor does it address how this timeline relates to other types of dementia. Lastly, the diffusion tensor imaging analysis was restricted to a smaller subset of patients, which might limit the generalizability of these specific predictive values.
