How do AF comorbidity patterns change care and outcomes?

In 32,560 AF patients, six comorbidity phenotypes ranged from low complexity to high complexity, with cardiometabolic/high-complexity groups facing higher event risk despite more anticoagulation.

Key findings

Six phenotypes: low complexity (39.2%), CV risk factors (28.2%), atherosclerotic (10.2%), thromboembolic (8.1%), cardiometabolic (7.6%), high complexity (6.6%). More complex groups used more OACs; cardiometabolic and high-complexity groups had the highest primary-outcome HRs (~1.37 and ~1.47).

Methodology

Analyzed the Prospective Global GLORIA-AF Registry to cluster comorbidity phenotypes and relate them to oral anticoagulant/rate-control use and long-term primary outcomes.

Limitations

Registry observational associations cannot prove that changing phenotype classification improves outcomes; residual confounding and regional practice variation remain.
