Short answer

Reduced mPFC activity during theory of mind statistically linked social anhedonia to worse social functioning in a small schizophrenia case-control study.

What happened

Dodell-Feder, Tully, Lincoln and Hooker compared 20 patients and 18 controls during a theory-of-mind task. Schizophrenia was associated with reduced mPFC activation. Lower mPFC activity related to poorer emotion management, perspective-taking, and social functioning. Mediation: reduced mPFC activity accounted for the link between high social anhedonia and worse social functioning.

Why it matters

This is not a new lesion story. It ties a social-cognitive network node to a clinical complaint (anhedonia) and to function. Cross-sectional data cannot say which way causality runs.

    People with schizophrenia show lower activity in a key brain region during perspective-taking, and this reduction explains why social anhedonia leads to worse everyday social functioning.

    What it does not show

    Because this study is cross-sectional, it cannot prove whether social anhedonia causes brain alterations or if brain differences drive social withdrawal. The study also had a relatively small sample size (consisting of 20 patients and 18 controls), which limited the power to detect correlations within each group individually. Finally, the schizophrenia participants had higher-than-average cognitive scores and IQs, meaning these results may not generalize to lower-functioning individuals with the illness.
