How the brain's perspective-taking network links to social loss

People with schizophrenia show lower activity in a key brain region during perspective-taking, and this reduction explains why social anhedonia leads to worse everyday social functioning.
Key findings
Compared to controls, individuals with schizophrenia showed significantly less activation in the medial prefrontal cortex during theory of mind reasoning. Across all participants, lower activity in this region was linked to poorer emotion management, lower perspective-taking, and worse social functioning. Crucially, mediation analysis showed that reduced medial prefrontal cortex activity accounted for the relationship between high social anhedonia and worse social functioning.
Methodology
Researchers compared 20 individuals with schizophrenia or schizoaffective disorder to 18 healthy control participants. While undergoing fMRI, participants completed a False-Belief Task involving 10 stories per condition about outdated beliefs or physical objects to isolate theory of mind processes. They also completed clinical, self-report, and performance-based tests of social anhedonia, empathy, emotion management, and overall social functioning.
Limitations
Because this study is cross-sectional, it cannot prove whether social anhedonia causes brain alterations or if brain differences drive social withdrawal. The study also had a relatively small sample size (consisting of 20 patients and 18 controls), which limited the power to detect correlations within each group individually. Finally, the schizophrenia participants had higher-than-average cognitive scores and IQs, meaning these results may not generalize to lower-functioning individuals with the illness.
