Authors obtained arthroscopic targeted synovial biopsies from 12 early RA patients (<1 year) and 4 long-standing RA patients, then compared histopathology with gene-expression clustering.

Eighteen samples from 16 cases formed two major expression groups. Group II vs I showed 180 genes up and 235 down (Q<5%), with significant histological score differences between groups — evidence that early RA synovitis is not one molecular state.
The limits
What it doesn't show
Two clusters in a small biopsy series do not define treatment arms. Arthroscopic knee samples may not represent every joint. Expression differences are associative with histology, not proven drug targets by themselves.
