What they did
Retrospective UK Biobank cohort of 15,104 adults with type 2 diabetes and no baseline macro/microvascular complications (2006–2010). Five lifestyle factors (waist circumference, smoking, physical activity, diet, alcohol) formed a 0–5 score. Outcomes ascertained from hospital/death records; Cox models adjusted for demographics, hypertension, HbA1c, and medications; mediation via blood biomarkers.
What they found
1,296 composite microvascular events over median 8.1 years. HRs for 4–5 vs 0–1 low-risk behaviors: 0.65 retinopathy, 0.43 kidney disease, 0.46 neuropathy, 0.54 composite (model 3). Each additional low-risk behavior lowered composite risk ~18%. PAF for <4 behaviors was 25.3% composite and 39.0% kidney disease. Albumin, HDL-C, triglycerides, apoA, CRP, and HbA1c jointly mediated 23.20% of composite association.
