Does reward system damage explain depression after a stroke?

Depression after a stroke is linked to microstructural changes and increased fluid in the brain's reward pathway rather than the physical location of the stroke's initial damage.
Key findings
The study found no clear link between the physical location of a stroke lesion and depression. Instead, patients with post-stroke depression exhibited a significant reduction in structural connectivity and an increase in extracellular free-water within a subnetwork covering up to 80% of the reward system. Structural abnormalities across these reward pathways—including the cingulum bundle and medial forebrain bundle—successfully predicted the severity of patients' depressive symptoms.
Methodology
The researchers evaluated 46 ischemic stroke survivors and 16 healthy controls using high-resolution MRI scans to map brain networks. They measured the severity of depressive symptoms using the 30-item Geriatric Depression Scale administered between 27 and 82 days after the stroke occurred. Using advanced imaging techniques, they examined the structural integrity and extracellular free-water content of white matter pathways in the brain's reward system.
Limitations
This study cannot establish causation, meaning it remains unclear whether these reward system changes existed before the stroke, resulted from the stroke indirectly, or share a common vascular cause. The sample size of 46 stroke patients is relatively small, which limited the researchers' ability to perform definitive voxel-based lesion symptom mapping. Additionally, because the study lacked a group of depressed patients who had not experienced a stroke, it cannot determine if the structural brain changes unique to post-stroke depression differ from those in typical major depressive disorder.
