Brain maps predict response to magnetic stroke therapy

Post-stroke patients whose brain activity peaks close to the stimulation target respond significantly better to magnetic brain stimulation.

Key findings

Patients whose pretreatment brain activation peaked within 25 mm of the stimulation target had an 86 % response rate, defined as a 5-point improvement or more on a motor recovery scale. In contrast, those with peak activity further away had only a 34 % response rate. For every 10 mm increase in this spatial distance, the odds of responding to the stimulation therapy dropped by 60 %. This spatial relationship did not predict motor recovery in patients who received standard rehabilitation alone.

Methodology

The researchers examined a cohort of stroke patients undergoing magnetic brain stimulation combined with rehabilitation, alongside a separate control cohort undergoing rehabilitation alone. Before starting the weeks-long treatment, the researchers recorded each patient's brain activity during wrist movement tasks. They measured the spatial distance from the standard stimulation site to each patient's actual peak brain activation point.

Limitations

The study cannot establish direct causality because it used an unmatched case-control design, which might contain unmeasured patient differences. Additionally, the research focused almost entirely on subacute stroke patients, so the findings may not apply to chronic stroke survivors. Because the researchers did not use precise MRI-guided neuronavigation, the exact real-time stimulation coordinates of the magnetic pulses were not recorded. Finally, the results are limited to a specific low-frequency stimulation protocol and might not generalize to other frequencies or alternative types of electrical brain therapies.
