Short answer

Addiction-related brain change is equally consistent with a developmental account of plasticity as with a disease account, and the imaging alone does not distinguish them.

What happened

Lewis argues that the self-perpetuating attraction seen in addiction is a feedback loop between experience and synaptic change, not evidence of disease. Brain-disease thinking, on this account, presupposes a neurotypical baseline to be repaired, whereas a developmental model treats the brain as an open system that can change in many directions — making recovery a further development rather than a cure.

Why it matters

Neural change is the observation both models share, so it cannot decide between them: any learned compulsion would produce it. What separates the accounts is what they say follows — whether recovery requires treatment of a pathology or the same plasticity that produced the problem — and that is a difference in framework rather than in data.
