How does the brain recognize emotions in music?

Damage to a distributed brain network, particularly in areas like the insula and amygdala, impairs a person's ability to identify emotions expressed in classical and film music.

Key findings

Patients with FTLD showed significant deficits in recognizing emotions across all modalities, with music and face recognition showing the highest power to discriminate patients from controls, yielding area under the curve values of 0.98 and 0.95 respectively. VBM analysis revealed that impaired music emotion recognition was associated with grey matter loss in a wide, bilateral network of brain regions including the insula, orbitofrontal cortex, anterior cingulate, and amygdala. This music-associated network was more extensive than the networks mapped for face or vocal emotion recognition, and remained significant even after controlling for executive function and general cognitive decline.

Methodology

Researchers tested 26 patients with frontotemporal lobar degeneration (FTLD)—specifically 16 with behavioral variant frontotemporal dementia (bvFTD) and 10 with semantic dementia (SemD)—alongside 21 healthy controls. Participants completed a four-alternative forced-choice task matching verbal labels for four emotions (happiness, sadness, anger, and fear) to auditory music clips, facial expressions, and vocal sounds. Voxel-based morphometry (VBM) was then used to analyze structural brain MRI scans and map grey matter loss associated with poor emotion recognition performance.

Limitations

The study's cross-sectional and correlational design using voxel-based morphometry cannot establish a direct, step-by-step causal pathway for how these brain regions process music dynamically in real time. The sample size of 26 patients is relatively small and limited to specific subtypes of frontotemporal lobar degeneration, meaning the findings might not generalize to other neurodegenerative conditions like Alzheimer's disease. Additionally, because the music clips utilized were classical and film score excerpts, the study cannot rule out the influence of prior familiarity or cultural exposure on the participants' emotional ratings.
